BUSINESS-TO-BUSINESS SERVICE AGREEMENT

1. PURPOSE OF AGREEMENT

This Agreement establishes a professional partnership in which Novum Medical & Sports Massage provides medical-based massage therapy services to support pain management, injury recovery, mobility, and stress reduction needs of the Partner’s employees, residents, guests, or members.

Medical-based massage therapy is therapeutic, targeted, and medically informed. Services are designed to address specific physical conditions rather than general relaxation.


2. SCOPE OF SERVICES

Provider may offer medical-based massage therapy services including, but not limited to:

  • Deep tissue and therapeutic massage
  • Trigger point therapy
  • Myofascial release
  • Neuromuscular therapy
  • Stretching and joint mobilization
  • Condition-specific treatment planning (as applicable)

Services may support treatment for:

  • Chronic and acute pain
  • Sports or work-related injuries
  • Repetitive stress and postural strain
  • Arthritis and joint conditions
  • Headaches and TMJ-related tension

3. PARTNER-SPECIFIC PROGRAM OPTIONS

Programs may be customized based on the Partner’s industry and workforce, including but not limited to:

Extended Stay / Hospitality Partners

  • Wellness support for long-term guests and staff
  • In-clinic treatment or optional on-site services
  • Preferred Pricing for Guests and Employees

Wellness Industry Partners

  • Cross-referral partnerships
  • Integrated recovery and care plans
  • Shared wellness education or workshops
  • Discounts apply to medical-based massage services (unless specifically agreed in writing).

4. PRICING & PARTNERSHIP DISCOUNTS

Standard Rate:

$125 per 50-minute Medical-Based Massage Session


Employer Partnership Discounts:

  • First Session: 50% off ($62.50)
  • Second Session: 20% off ($99)
  • Ongoing Sessions: 10–80% off

Discounts are based on participation level and visit frequency.


5. SCHEDULING & LOCATION

  • Services may be provided in-clinic or on-site
  • Scheduling coordinated directly with Provider
  • Minimum participation may apply for on-site services

6. TERM & TERMINATION

  • Either party may terminate with 30 days’ written notice
  • Immediate termination permitted for breach of agreement
